REQUIRED DOCUMENTS - INDIVIDUAL PLACEMENT
Before starting the individual practice, the student downloads the relevant documents (attachments) from the website.

Attachment No. 4 (files below) - Application for permission to organise the placement on your own. A completed scan of the application for the consent to organise the placement on your own together with the consent of the unit director should be sent by e-mail until 07.04.2023 to :firstname.lastname@example.org
appendix no. 2.1 (file below) - agreement with the entity to organise the practice on their own (2 copies)
The condition for the realization of the internship is the signing of an agreement between the University and the unit where the internship is to be realized. Within the agreement, the University provides students with third-party insurance, while students must purchase accident insurance individually.

ATTENTION: Please print the apprenticeship programme on both sides. Single-sided printouts will not be accepted for credit.

After completion of the internship, please send the internship PROGRAMS, signed and stamped by persons responsible for supervision of the internship on behalf of the entity to email@example.com until 22.09.2023 (students doing the internship in the second half of September to 06.10.2023).
