New statutory obligation for students
Dear Students, as of February 15, 2024, a new statutory obligation comes into force for students who are to be admitted, within the framework of their study program (clinical practical classes/practical classes with patients, student internships) to activities involving, among others, treatment, education or care for minors (i.e. persons under the age of 18).
Due to the amendments to the provisions of the Act of May 13, 2016, on Counteracting Sexual Crime (consolidated text: Journal of Laws of 2023, item 1304, as amended), students of: Medicine in Polish and English, Dental Medicine in Polish and English, Physiotherapy, Medical Rescue, Midwifery, Nursing, Public Health, Dietetics, Pharmacy (students from the 3rd year or above), and Medical Analytics (students from the 2nd year or above) must submit by February 26, 2024:
1. certificates from the National Criminal Register (KRK) - Polish citizens and foreigners,
2. statements about the country or countries where the student has resided in the last 20 years, other than the Republic of Poland and the country of citizenship, along with information from the criminal registers of these countries obtained for professional or voluntary activities involving contacts with children - Polish citizens and foreigners,
3. information from the criminal register of the country of citizenship along with sworn translation obtained for professional or voluntary activities involving contacts with children - foreigners.
No. 1 CERTIFICATE FROM KRK
You can obtain a certificate from KRK:

Please complete the application with your personal data and:
a) in point 11 please enter: "admission to activities involving treatment, education or care for minors within the framework of the study program",
b) in point 12 please mark: "criminal record",
c) in point 13, enter: "crimes specified in chapters XIX and XXV of the Penal Code, Art. 189a and Art. 207 of the Penal Code and the Act of July 29, 2005, on Counteracting Drug Addiction (Journal of Laws of 2023 item 172 and of 2022 item 2600), or prohibited acts corresponding to these crimes specified in the provisions of foreign law".

To electronically apply for a certificate from KRK, you must have a qualified electronic signature, a trusted signature, or a personal signature. Cost of issuing a certificate – PLN 20.
NOTE: The document is issued as an XML file. The document shall be sent to the e-mail address of the appropriate Dean's Office from the university student's e-mail address. The printout is not a document.

The declaration is submitted under the penalties of perjury for submitting a false declaration.
No. 3 DETAILED INFORMATION
1. How to proceed in a situation where the law of the country of citizenship or the country/countries of residence in the last 20 years, other than the Republic of Poland and the country of citizenship, does not provide for information issuance for professional or voluntary activities involving contacts with children?
Information from the criminal register of that country(ies) must be submitted along with sworn translation.
2. How to proceed in a situation where 1) the law of the country of citizenship, 2) the law of the country/countries of residence in the last 20 years, other than the Republic of Poland and the country of citizenship, from which information from the criminal register obtained for professional or voluntary activities involving contacts with children or information from the criminal register of that country shall be submitted, does not provide for its preparation or a criminal register is not maintained in that country?
You must submit a declaration (Appendix No. 2)
The declaration is submitted under the penalties of perjury for submitting a false declaration
What are the sanctions if I do not submit the legally required documents and declarations?
Pursuant to Art. 23 sec. 3 of the Act on Counteracting Sexual Crime Risks, as in effect from February 15, 2024, a person who, before being admitted to activities involving, among others, education, treatment or care for minors, does not fulfil the obligation to submit information or declaration referred to in Art. 21 sec. 3-7 of the Act, is subjected to imprisonment, restriction of liberty or a fine of not less than PLN 1,000.

WHERE SHOULD THE DOCUMENTS BE SUBMITTED?
Please submit your documents to the Dean's Office - The Faculty of Dentistry, English Division - or send your electronic versions via email to your supervisor at the Dean’s Office.
The required documents must be submitted by February 26, 2024.
