IMPORTANT  INFORMATION  FOR  YOUR  CONSIDERATION: 

A) University Email Account

Please go to https://csa.umw.edu.pl/#/ and set up your university email account with the domain student.umw.edu.pl.

Your email address is needed to contact your professors, access the library electronic resources or complete the required training online.

B) Virtual University

The website to download referral forms for medical examinations, view your details, check your account numbers, grades, your study history, subjects you have been registered for, etc.
Please visit:  https://wu.umw.edu.pl – the login details and your password are exactly the same as the ones you have used to set up your university email account. Note: your login is as follows: name.surname without Polish characters.

C) Student Identity Card

The fee for the student identity card is 22 PLN. It should be paid by 31st October 2023 to an individually generated bank account. The number can be found in the "Agreement on the conditions of payment for educational services and other payments related to higher education" (§ 4). 

D) Medical Examinations

You are required to get two medical examinations done and the referral forms for the medical tests can be collected from the Dean's Office:

• The Medical Examination (free of charge).  Please find the list of the facilities attached at the bottom of this website.
• The Sanitary and Epidemiological Examination.

The Occupational Health Assessment may be completed free of charge at the medical centres provided by the local occupational health centre in Wroclaw: Dolnośląski Wojewódzki Ośrodek Medycyny Pracy in Wrocław. The original certificate from your medical test should be submitted to the Dean's Office, English Division by 27th October 2023.

Please note that it is not the same certificate which was required at the recruitment process. The above mentioned examinations must also be performed in Poland.

The Sanitary and Epidemiological Examination should be performed at the microbiological laboratory: Laboratorium Mikrobiologii i Parazytologii Wojewódzkiej Stacji Sanitarno-Epidemiologicznej in Wrocław at Składowa 1/3.
The stool (faeces) sample should be submitted from Monday to Wednesday from 8:00 a.m. to 11:00 a.m. (Punkt Przyjmowania Próbek Oddziału Mikrobiologii WSSE in Wrocław, ul. Składowa 1/3).

Instructions: The three stool samples must be collected on three consecutive days by using the stool containers, which should be bought at a chemist's or collected from the Punkt Przyjmowania Próbek, Wrocław, ul. Składowa 1/3. Please note that it is important to get the 3 swabs with the Stuart, Cary-Blair or Amies transport medium. Otherwise, your samples will not be accepted by the lab.

All collected stool samples must be delivered together, no later than 72 hours after taking the first sample, to the Punkt Przyjmowania Próbek Oddziału Mikrobiologii WSSE in Wrocław (ul. Składowa 1/3).

The hard copy of the test result can be collected 5 days after the delivery of your samples at the Punkt Przyjmowania Próbek Oddziału Mikrobiologii WSSE in Wrocław (ul. Składowa 1/3) from: Monday to Friday between 8:00 a.m. and 3:00 p.m. or via the website after logging in with the login details received when ordering the test on the platform https://kl941.elaborat.marcel.pl/

Finally, the test results should be presented to the occupational physician and a paid medical certificate of the sanitary and epidemiological test should be obtained. The certificate should be submitted to the Dean's Office, English Division by 27th October 2023.

In addition to the above certificates, each student must also submit the certificate of the hepatitis vaccination by 27th October 2023 to the ED Dean's Office.

E) Occupational Health and Safety and Fire Protection Training and Test

The training on the principles of the Occupational Health and Safety and Fire Protection will be conducted online and will last 4 hours. It will be held on 16th & 17th October 2023.

To take the test, please ensure youset up your e-mail address first with the student.umw.edu.pl domain, because an activation link for the training will be sent to this e-mail address.

The training will end with a test that each student is required to take. To pass the test, 80% of the questions are required to be answered correctly. You can scroll through the test, return to previous questions and slides.

Failure to pass the Health and Safety and Fire Protection Training may result in removal from the list of students.
