To all students from English Division that shall conduct their apprenticeship in Wroclaw!

You are requested to conduct the necessary health tests: sanitary one based on stool sample and by occupational practitioner at least one month before You start the apprenticeship.
If you did not collect the required referral you are asked to do it as soon as possible in ED Office.
On the first day of the apprenticeship you should have:
-  the vaccination booklet for Hepatitis type b,
- sanitary booklet with the stamp of the occupational practitioner,
- certificate issued by the occupational practitioner
- accident insurance,
- apprenticeship programm, blue index
- protective apron and schoes for change.
The head of every facility you shall enter has the right to ask you to show the full set of the documents and not to allow you enter if you shall not have it.
