Nomination procedure: 

1. All nominations should be sent to: dominika.lucinska@umw.edu.pl , including those submitted via the dashboard.
2. Deadline for submitting nominations: 15.05.2026

To prepare a correct Learning Agreement, please use only the curriculum  for the 2026/2027 academic year (available soon). Please ensure that the number of ECTS credit points corresponds to the requirements specified in the curriculum 2026/2027.

Sign up and log in to on-line application form, follow the link:

 1.Fill in the form in 'My application form'.

2.Add photograph in 'My profile' tab (add official photo like for ID card/passport).

3.Add Learning Agreement for studies https://learning-agreement.eu/  and upload PDF with signatures: Student, Faculty Coordinator of Home University,
Learning Agreement  for traineeships signed by you and Erasmus+ coordinator from your University

After you successfully upload the documents from the SECOND STEP, you will be able to upload the remaining required documents.

ERASMUS-STUDIES
Upload the remaining documents by 31 August 2026 for the 1st semester and full-year stays, and by 31 December 2026 for the 2nd semester.

• scan of id card (or passport)
• scan of European Health Insurance Card or any relevant one
• scan of original confirmation of accident insurance
• scan of original liability insurance (mandatory for traineeships, optional for studies)
• scan of Erasmus+ status certificate (issued by your home university)
• scan of student's statement

Come to the  Erasmus Office personally (bring with you confirmation of payment for student card).
When your student's card is ready you will be informed about the date to collect it.

Please make sure that upon arrival you have got:

• the proof of payment 22 zl for students Id

• you should apply for criminal record certificate: certificate from Polish National Criminal Register (KRK) and sign appendix nr 1(to be done upon arrival, not earlier).
