Learning Agreement for studies https://learning-agreement.eu/  and upload PDF with signatures: Student, Faculty Coordinator of Home University,
Learning Agreement  for traineeships signed by you and Erasmus+ coordinator from your University 
Please send above listed document before deadline:

• 31 May 2024 -  for winter semester, for whole academic year
• 30 November 2024 – for summer semester

• scan of id card (or passport)
• scan of European Health Insurance Card or any relevant one
• scan of original confirmation of accident insurance
• scan of original liability insurance (mandatory for traineeships, optional for studies)
• scan of Erasmus+ status certificate (issued by your home university)
• scan of student's statement

Please make sure that upon arrival you have got:

• the proof of payment 22 zl for students Id

• you should apply for criminal record certificate: certificate from Polish National Criminal Register (KRK) and sign appendix nr 1
